Indications/Uses
Dosage/Direction for Use
Individualized premed. Induction Initially 0.5%. Surgical anaesth 1.5-3% in 7-10 min. Maintenance 1-2.5% in O2/nitrous oxide mixt. Additional 0.5-1% when given w/ O2 alone. Recovery May be reduced to 0.5% near end of surgery or to 0% at the beginning of skin closure.
Contraindications
Hypersensitivity to isoflurane or other halogenated anesth. Known or suspected genetic susceptibility to malignant hyperthermia.
Special Precautions
Transient rise in CSF pressure; hypotension & resp depression; patients at risk for QT prolongation; patients undergoing induced abortion; increased ICP, mitochondrial disorders. Patients who can develop bronchoconstriction. Malignant hyperthermia. Perioperative hyperkalemia, Duchenne muscular dystrophy. Discontinue use of IV dantrolene Na & application of supportive therapy. Augmented resp depression w/ narcotic premed or other resp depressants. Increased carboxyhemoglobin w/ concurrent use of fluorinated inhalation agents. Decrease intellectual function for 2-4 days following anaesth. May affect ability to drive & use machines. Pregnancy & lactation. Childn <2 yr.
Adverse Reactions
Drug Interactions
Increased serum K levels w/ succinylcholine. Ventricular arrhythmia w/ β-sympathomimetic agents eg, isoprenaline, α- & β-sympathomimetic agents eg, adrenaline & noradrenaline. Risk of crisis w/ non-selective MAOIs. Increased plasma fluoride conc w/ INH, alcohol. Increased risk of potentiated hepatotoxic effects w/ INH. Marked hypotension w/ Ca antagonists eg, dihydropyridine derivatives. Risk of additive -ve inotropic effect w/ inhalation anaesths. Resp depression w/ opioids, benzodiazepines & other sedative agents.
